Role of Dynamic Magnetic Resonance Imaging in the T-Staging of Advanced Stomach Cancer

Abstract

|
Background/Aims : We evaluated the usefulness of the dynamic and delayed magnetic resonance (MR) imaging in the T staging of advanced stomach cancer.
Methods : Authors performed the MR imaging in 42 patients with advanced gastric cancer. Two dimension-fast low-angle shot (2D-FLASH) MR images for the water-distended stomach were obtained with dynamic protocol after intravenous injection of the 0.1 mM Gd-DTPA (gadopentetate dimeglumine) /§¸ solution. We prospectively evaluated the depth of cancer invasion, perigastric infiltration (extraserosal invasion), perigastric organ invasion. We correlated the MR-determined T-staging with the surgicopathologic findings.
Results : The mucosa (or with submucosa) invaded by stomach cancer showed early enhancement pattern in 39 patients (93%) 30 to 90 seconds after Gd-DTPA administration. The normal gastric mucosa demonstrated delayed peak enhancement pattern in 24 patients (57%) or variable enhancement pattern in 18 patients (43%). Interrupted low signal intensity band or highly-enhanced tumorous lesion penetrating through gastric wall were observed in 17 of 19 pT3 patients (90%). Consistency between MR-determined stage and surgicopathologic staging was observed in 10 of 13pT2 tumors (77%), 17 of 19 pT3 tumors (90%), and 8 of 10 pT4 tumors (80%). The overall accuracy was 83%.
Conclusions : Dynamic MR imaging can be useful for T-staging of advanced gastric cancer.
